The System is intended solely for legitimate medical purposes and may not be used for any unauthorized commercial activities.
As an authorized user of the System, you bear full responsibility for maintaining the accuracy and completeness of all information submitted during the appointment booking process. You must ensure that all personal and health data provided is current and correct. Any misuse of the System, including but not limited to unauthorized access attempts, data scraping, or disruption of services, constitutes a violation of these Terms and may result in immediate account termination and potential legal action under relevant Philippine cybercrime laws (RA 10175).
Pangasinan Doctors Hospital maintains strict compliance with the Philippine Data Privacy Act of 2012 (RA 10173) in all aspects of personal health information management. These Terms and Conditions shall be exclusively governed by and construed in accordance with the laws of the Republic of the Philippines, without giving effect to any principles of conflicts of law. Any legal action or proceeding arising under or relating to these Terms shall be brought exclusively in the proper courts of Dagupan City, Pangasinan, and you irrevocably consent to the personal jurisdiction of such courts. The prevailing party in any legal action to enforce these Terms shall be entitled to recover reasonable attorneys' fees and costs in addition to any other available remedies.
